I, (parent/guardian), give my son/daughter permission to go to the Districts Youth Conference in Green Bay WI from Friday, January 15th through Sunday, January 17th, 2026. I understand that the cost for the Conference is $135.00 (The registration deadline is November 4 as space allows) I understand I can pay now with Credit/Debit Card or pay by November 15th with check or cash.
If your family cannot afford the $135 registration fee in part or whole, I understand that I can contact Pastor Trevor to get a scholarship to help defer the cost of this trip.
We know being away from home can cause anxiety, but we always want to try to help students feel as comfortable as possible while on a trip. One of the ways to do that is to be able to room with people they know and like. We will try our best to pair your child with as many friends as possible but will guaruntee one friend in their room if that friend requested is going. Please choose 3 friends that are going to possibly room with.

In case of an emergency where we cannot get ahold of your child's emergency contact we will need to know the Insurance information. Please add this info for our records
I understand that the event will take place at The KI Covention Center in Green Bay, WI. I understand that while there, my son/daughter may choose to use the pool at the hotel we will most likely be staying at. My child will have experiences associated with a youth conference and indoor pool. Should an accident or injury of any kind occur while my son/daughter is participating, I release each church represented at the conference, along with their pastors, boards, directors, leaders, sponsors and all additional adult chaperones on the trip from any responsibility or liability for the accident or injury and from any claim for damages from the accident or injury. In the event that emergency medical treatment is required, I give my permission to the medical personnel to provide treatment. I understand that I will not hold any denomination or church represented at the retreat responsible for any charges incurred during emergency medical treatment and that I, myself, as parent or legal guardian, am solely responsible.

Please make checks out to Sonlife. All registration fees need to be paid in full by November 15th.
We at Sonlife never want funds to be an issue if a student can go to an event or not so we have generous folks who would be willing to scholarship the needed funds for students to attend. Please contact Pastor Trevor if you desire a scholarship and he will make arrangements.
